Späte Diagnose einer myotonen Dystrophie Curschmann-Steinert bei einer Patientin mit Dilatativer Kardiomyopathie und ihrem Sohn

Summary

A 41 year old woman presented with dyspnoea at rest and swollen legs in the emergency room of our centre. She reported a history of slowly progressing dyspnoea and oedema in the legs. Physical examination showed signs of biventricular congestive heart failure and dysmorphia of the face. Routine laboratory examination revealed elevated CK levels without significant elevations of the CK-MB isoform. ECG showed complete left bundle branch block and first degree atrioventricular block. Echocardiography and angiography showed markedly reduced left ventricular systolic function, the ejection fraction was 25%. Coronary angiography excluded CAD and there was no evidence for congenital or valvular heart disease. The patient also reported a history of a serious complication during emergency general anaesthesia and cataracts of both eyes.    Because of the clinical and chemical findings, the history of cataracts and complications during general anaesthesia, a systemic congenital disease of the muscular tissue was suspected. Molecular studies revealed a trinucleotide amplification at the myotonic dystrophy locus 19q13.3, so the diagnosis myotonic dystrophy Curschmann-Steinert was established. The sixteen year old son of the patient suffered from an at this time unknown disease with retardation, muscular weakness and myotonia of the face. The diagnosis myotonic dystrophy was evident because of the clinical signs and the family history.

Zusammenfassung

Eine 41-jährige Patientin stellte sich mit Ruhedyspnoe und geschwollenen Unterschenkeln in der Notaufnahme unseres Zentrums vor. Sie berichtet über eine langsam progrediente Dyspnoe und zunehmende Ödeme der unteren Extremitäten. Die körperliche Untersuchung erbrachte Zeichen der biventrikulären Herzinsuffizienz sowie deutliche faciale Auffälligkeiten. Im EKG zeigte sich ein kompletter Linksschenkelblock. Laborchemisch imponierte eine Erhöhung der Gesamt-CK ohne CK-MB-Anteil. Echocardiographie und Laevocardiographie zeigten eine deutlich reduzierte linksventrikuläre Pumpfunktion mit einer Ejektionsfraktion von ca. 25%. Die Koronarographie war unauffällig. Ein kongenitales oder erworbenes Vitium als Ursache der Ventrikelschädigung konnten ausge-schlossen werden. Die Patientin berichtete im weiteren Verlauf von einem stattgehabten Narkosezwischenfall sowie von einer beidseitigen Katarakt.    Aufgrund der klinischen und biochemischen Befunde und der Anamnese mit beidseitiger Katarakt und Narkosezwischenfall wurde in erster Linie an eine hereditäre Muskelerkrankung, insbesondere an eine myotone Dystrophie gedacht. Molekulargenetisch wurde der Nachweis der Repeatexpansion im Bereich von 19q13.3 (CTG-Amplifikation) erbracht und damit die Diagnose eines Morbus Curschmann-Steinert gesichert. Der Sohn der Patientin litt unter einer bislang unbekannten Erkrankung mit Retardierung, Muskelschwäche und Gesichtsauffälligkeiten. Aufgrund des Phänotyps und des autosomal-dominanten Erbgangs der Erkrankung wurde auch bei ihm die Diagnose myotone Dystrophie Curschmann-Steinert gestellt.
