Zusammenfassung
Der segmentale Hodeninfarkt ist ein seltenes Ereignis und äußert sich klinisch als akutes Skrotum. Als wichtige Differenzialdiagnosen müssen eine Hodentorsion, Epididymorchitiden und Hodentumoren in Betracht gezogen werden. Bis heute sind in der Literatur nur wenige Fälle beschrieben worden. Im aktuellen Artikel berichten wir über einen 61-jährigen Patienten, der sich mit seit 24 h bestehenden linksseitigen Hodenschmerzen in unserer urologischen Ambulanz vorstellte. In der Sonographie mit ergänzender farbkodierter Dopplersonographie wurde der Verdacht auf einen segmentalen Hodeninfarkt gestellt, der sich bei der inguinalen Hodenfreilegung bestätigte. Der segmentale Hodeninfarkt soll bei Vorliegen einer klinischen Symptomatik in die differenzialdiagnostischen Überlegungen des akuten Skrotums mit einbezogen werden, insbesondere bei jungen Patienten, da das therapeutische Vorgehen organerhaltend sein kann.
Abstract
Segmental testicular infarction is a rare cause of acute scrotum and only a few cases have been reported. Torsion of the testis, testicular tumor and infection are important differential diagnoses. The present case report describes a 61-year-old man with left-sided testicular pain increasing over 24 h. The diagnosis of segmental testicular infarction was considered after color Doppler ultrasound of the left scrotum and it was confirmed by surgical exploration and pathological examination. Although it is uncommon, segmental testicular infarction should be taken into consideration when acute scrotal pain is encountered, especially for younger patients, since a testis-sparing treatment strategy can be performed.
