Zusammenfassung
Die partielle Schwellkörperthrombose ist eine sehr seltene Erkrankung, die v. a. bei jüngeren Männern auftritt. Das Hauptsymptom ist eine schmerzhafte perineale Schwellung bei erhaltener Erektionsfähigkeit. Die genaue Pathogenese ist nicht bekannt. Die diagnostische Methode der Wahl ist die Magnetresonanztomographie (MRT). In der Therapie der partiellen Schwellkörperthrombose hat sich bei guten Langzeitergebnissen in den letzten Jahren ein Wandel von den operativen Verfahren hin zu konservativen Behandlungsansätzen mit Antikoagulation und Analgetika vollzogen. Wir berichten erstmals über das Auftreten einer partiellen Schwellkörperthrombose bei Homozysteinämie. Eine tabellarische Übersicht stellt die in der Literatur dokumentierten Fälle hinsichtlich Pathogenese, Diagnostik, Therapie und Verlauf dar.
Abstract
Partial unilateral penile thrombosis of the corpus cavernosum is a very rare disease and occurs mainly in young men. The leading symptoms are perineal swelling and pain, while erectile function is maintained. The exact pathogenesis of this condition remains unclear. This is the first report of partial penile thrombosis due to hyperhomocysteinemia. Magnetic resonance imaging is the procedure of choice in diagnostics of partial penile thrombosis. For the treatment of partial penile thrombosis, surgical procedures are increasingly being replaced by more conservative treatments with analgesics and heparinization. Both surgical and conservative treatment provides excellent outcomes. A table provided in this article lists all published cases and refers to pathogenesis, diagnostics, therapy, and follow-up.
