Virale en bacteriële lageluchtweginfecties zijn in de dagelijkse praktijk niet van elkaar te onderscheiden

Samenvatting

Doel Indien virale en bacteriële lageluchtweginfecties (LLWI) in de dagelijkse praktijk van elkaar onderscheiden kunnen worden, kan dit leiden tot een vermindering van onjuist en onnodig voorschrijven van antibiotica.
Methode Vijfentwintig huisartsen registreerden klinische gegevens van 247 volwassen patiënten die op het spreekuur kwamen met een LLWI. Sputum, mondspoeling, neus-keelwat en bloedmonsters werden onmiddellijk na het consult afgenomen en geanalyseerd op virale en bacteriële verwekkers, BSE, CRP en leukocyten. De voorspellende waarde van de klinische gegevens en BSE, CRP en leukocytenaantal voor een virale infectie werd bepaald met behulp van logistische regressieanalyse.
Resultaten Een virale, bacteriële of gemengd virale-bacteriële infectie werd vastgesteld bij 112 van de 234 patiënten voor wie volledige gegevens beschikbaar waren (48%). Virale pathogenen bleken even vaak aanwezig als bacteriële. Haemophilus (para-)influenzae werd het vaakst aangetroffen. Geen van de onderzochte klinische gegevens vertoonde een statistisch significante correlatie met virale of bacteriële lageluchtweginfectie. Onafhankelijke voorspellers van microbiologisch verklaarde (virale of bacteriële) LLWI ten opzichte van microbiologisch onverklaarde LLWI waren BSE >50 (OR 2,3-3,3) en CRP >20 (OR 2,1-4,6).
Conclusie Virale en bacteriële LLWI bij volwassenen zijn in de dagelijkse huisartsenpraktijk niet van elkaar te onderscheiden.
