Summary

e-Health – health care through the internet

e-Health, the employment of information and communication technology for health care, offers promising opportunities for quality enhancement and cost containment of health care delivery. In this paper we accumulate evidence for the efficacy of internet interventions designed to promote a healthy lifestyle (such as weight control and low risk alcohol consumption), to reinforce self-management of chronic somatic conditions (such as headache), and to overcome mental health problems (such as depression and anxiety disorders). Evidence suggests that internet interventions may be equally effective as regular face-to-face interventions. Current research aims at cost effectiveness, at mobile devices for online monitoring and coaching, at successful components of online-treatment, and at effective implementation strategies. We identified problems such as e-therapy adherence and challenges such as the delivery of e-health care per target population based upon a stepped care approach. We conclude that e-health is here to stay and will fully integrate into all aspects of health care delivery.

  1. Een aanverwante term is ‘telemedicine’ gericht op zorg op afstand (telezorg) via ICT, internet, sensoren, intelligente televisie, alarm- en beslissingsondersteunende systemen en andere technologie in de thuissituatie van chronisch zieken en ouderen. De term wordt toenemend onder e-health geschaard (European Commission, Information Society, 2009; Van Thiel, 2009). Dit gebied van e-health blijft in dit artikel buiten beschouwing.

Het ging om ‘Interactive Health Communication Applications’ (IHCAs) bij volwassenen op de gebieden van AIDS/HIV (N = 2), de ziekte van Alzheimer en geheugenverlies (N = 2), obesitas (N = 2), eetstoornissen (N = 1) en incontinentie (N = 1); bij kinderen en volwassenen op de gebieden van kanker en leukemie (N = 2) en diabetes (N = 2), en bij kinderen op de gebieden van astma (N = 6) en encopresis (N = 1).

  3. Het betrof 14 internetinterventies voor de geestelijke gezondheidszorg [angststoornissen, waaronder paniek, PTSS en sociale angst (N = 8), depressie (N = 3) en boulimie en lichaamsbeeld (N = 3)], 9 studies voor de somatische gezondheidszorg [hoofdpijn (N = 2), lage rugpijn (N = 1), diabetes (N = 1), encopresis (N = 1), tinnitus (N = 3) en slapeloosheid (N = 1)] en 3 studies op het gebied van leefstijlinterventie [gewichtscontrole (N = 2) en stoppen met roken (N = 1)].
