Complicaties bij pilon-tibialfracturen en gevolgen voor patiënten in Ziekenhuis Rijnstate Arnhem (2001-2006)

Samenvatting

Doel. Onderzoek naar de complicaties op korte en lange termijn bij pilonfracturen in combinatie met een subjectieve beoordeling door de patiënten zelf.Opzet. Retrospectief.Methoden. Er zijn gegevens verzameld van 16 patiënten met 17 pilonfracturen die zijn geopereerd tussen januari 2001 en februari 2006 in het Rijnstate Ziekenhuis in Arnhem. Alle complicaties zijn geregistreerd. Bij 13 patiënten is daarnaast een vragenlijst afgenomen waarbij is gevraagd naar sport, werk en subjectieve beleving.Resultaten. De operatieve technieken die zijn gebruikt in onze studie: open reductie en interne fixatie (ORIF), fixateur externe en een combinatie van ORIF en fixateur externe. Bij 12 patiënten ontstonden er postoperatief complicaties. Infecties van de weke delen kwamen bij 3 (18%) van de 17 pilonfracturen het meest voor als kortetermijncomplicatie. Als late complicaties kwamen artrose en mal-unions het meest voor, respectievelijk bij 3 (18%) en bij 4 (24%) van de 17 pilonfracturen. Er zijn tussen de diverse technieken geen verschillen gevonden in het optreden van complicaties op korte en lange termijn. Op basis van de vragenlijst hebben zeven patiënten een matige tot slechte uitkomst. Vijf patiënten konden hun oude werkzaamheden hervatten en vier patiënten konden uiteindelijk weer sporten.Conclusie. Er zijn geen verschillen gevonden tussen de operatieve behandelingen in deze studie. De impact van een pilonfractuur op het functioneren van patiënten is aanzienlijk.
