Onwaarschijnlijke symptomen simuleren: de Wildman Symptom Checklist

Samenvatting

Patiënten die symptomen simuleren bestaan. Maar hoe spoor je ze op? De consensus is dat de clinicus er verstandig aan doet om meerdere instrumenten in te zetten, waaronder tests en taken, maar ook een interview. In zo'n interview moet worden nagegaan of de patiënt ertoe neigt om voor de dag te komen met onwaarschijnlijke symptomen en kenmerken. Maar wat vraag je dan aan de patiënt? In ons onderzoek probeerden we bruikbare symptomen en kenmerken te genereren.

Ook de door Rogers en collega's (1992) ontwikkelde sirs bestaat uit een lange reeks van onwaarschijnlijke, atypische en non-existente symptomen en kenmerken. De rationale achter de sirs is dat simulanten zullen zeggen last te hebben van dit soort symptomen.

Een integrale vertaling van de checklist en realistische items is bij de auteurs te verkrijgen.

De getalsmatige verhouding tussen controles en simulanten is niet toevallig: zij is zo gekozen dat de base rate van simulatie in de buurt komt van de door Greve en Bianchini (2004) aanbevolen 0.30.
