Propyleenglycol, een verraderlijk oplosmiddel in geneesmiddelen

Samenvatting

Bij een asfyctisch geboren jongen, 3.985 gram, bij wie fenobarbital als anticonvulsivum en lorazepam intraveneus als sedativum werden gebruikt, ontwikkelden zich hypertrofische cardiomyopathie met circulatoire insufficiëntie en ritmestoornissen, convulsies en hypoglykemieën en werden verhoogde leverenzymen aangetoond. Ondanks uitgebreid viraal en metabool onderzoek werd aanvankelijk geen oorzaak gevonden. Door een sterk verhoogde lorazepam-spiegel (444 μg/l, therapeutisch 20-250 μg/l) rees het vermoeden op intoxicatie. Hoewel de patiënt volledig herstelde na het staken van lorazepam vonden we het klinisch beeld niet volledig compatibel met lorazepam-intoxicatie. Omdat als oplosmiddel voor fenobarbital en lorazepam voor intraveneuze toediening propyleenglycol wordt gebruikt, bepaalden we ook de propyleenglycol-concentratie die twee dagen na het staken van de lorazepam nog 65 mg/l bleek te zijn. Intoxicatie met propyleenglycol kan cardiale, neurologische en metabole symptomen veroorzaken. Propyleenglycol wordt gebruikt om lipofiele geneesmiddelen oplosbaar te maken voor intraveneuze toediening. In het bijzonder patiënten met verminderde lever- en/of nierklaring hebben een verhoogd risico op intoxicatie met propyleenglycol. Bij elke patiënt met een onbegrepen klinisch beeld en verhoogde osmolal gap die propyleenglycolbevattende medicatie krijgt moet gedacht worden aan de mogelijkheid van propyleenglycol-intoxicatie.
