Drop-outs bij de behandeling van hypochondrie: kenmerken en aanpak

Samenvatting

Kenmerken van patiënten die voortijdig hun behandeling stoppen (drop-outs) zijn nog weinig onderzocht. Veelal worden deze patiënten gezien als hopeloze gevallen. Recent onderzoek naar patiënten met hypochondrie laat een ander beeld zien. Drop-outs hebben minder ernstige klachten dan patiënten die de behandeling afmaken, en ze zijn vaker nog aan het werk. Mogelijk maken zij een kosten-batenanalyse, komen tot de conclusie dat de kosten niet opwegen tegen de baten en stoppen vervolgens de behandeling. Het lijkt de moeite waard deze patiënten in behandeling te houden. Voortijdig stoppen kan bijvoorbeeld worden voorkómen door een helder en expliciet behandelingscontract, motiveringstechnieken, aanpassingen in het behandelplan en praktische afspraken.

Abstract

Characteristics of patients who prematurely stop their treatment (drop-outs) have rarely been investigated. These patients are mostly considered as hopeless cases. For hypochondriacal patients, recent research reveals a different picture. Drop-outs have less complaints, and often still go to work. Perhaps they make a cost-benefit analysis of treatment, arrive at the conclusion that costs do not compensate benefits and stop treatment. It is worthwhile to keep these patients in treatment. Prematurely quitting treatment can be prevented, i.e by a clear and explicit treatment-contract, motivating techniques, adjustment of the treatment-plan and practical arrangements.
