Abstract
In a prospective longitudinal study of 354 parents and their first-born infants the association between parental psychopathology and risk for maladjustment in the offspring was investigated. At age two infants of disturbed parents displayed significantly poorer language performance and had higher scores of behaviour problems as compared to infants of healthy parents. While maternal disorder affected both cognitive and social-emotional outcome, the impact of paternal disorder was confined to cognitive functioning. The poorest outcome was found in infants of mothers with a personality disorder or with alcohol abuse. The relationship was maintained even after accounting for a number of family risk factors associated with parental psychopathology. One mediating factor between maternal disorder and infant maladjustment was found in disturbed mother-infant interaction. Maternal mental health did not only affect infant outcome, but was itself influenced by infant adaptation.
Résumé
Dans une étude prospective longitudinal de 354 parents et de leurs enfants premiers nés l’association entre la psychopathologie parentale et le risque d’inadaptation de la descendance a été recherchée. A deux ans les enfants de parents présentant des troubles montraient des performances de langage significativement plus pauvres et avaient des scores plus élevés de problèmes comportementaux comparés aux enfants de parents sains. Tandis que la pathologie maternelle affectait à la fois le devenir cognitif. Les devenirs les plus pauvres furent trouvés chez les enfants de mère avec une pathologie de la personnalité ou un alcoolisme. La relation était maintenue même en tenant compte de facteurs de risque familiaux associés à la psychopathologie parentale. Le trouble de l’interaction mère-enfant a été reconnu comme un facteur de médiation entre la pathologie maternelle et les difficultés de l’enfant. La santé mentale de la mère non seulement affectait le devenir de l’enfant, mais était elle-même influencée par l’adaptation de l’enfant.
Zusammenfassung
In einer prospektiven Längsschnittstudie mit 354 Eltern und ihren erstgeborenen Kindern wurde der Zusammenhang zwischen psychischen Störungen der Eltern und Beeinträchtigungen der kindlichen Entwicklung untersucht. Im Alter von zwei Jahren waren die Kinder psychisch auffälliger Eltern vor allem sprachlich weniger weit entwickelt und in ihrem Sozialverhalten auffälliger als die Kinder gesunder Eltern. Während sich die Auswirkungen der psychischen Auffälligkeit der Mutter sowohl in Beeinträchtigungen der kognitiven wie der sozialemotionalen Entwicklung manifestierten, ging die Auffälligkeit des Vaters allein mit Abweichungen der kognitiven Entwicklung einher. Die ungünstigsten Entwicklungskennwerte fanden sich bei Kindern, deren Eltern unter einer Persönlichkeitsstörung oder einer Suchtproblematik litten. Der Zusammenhang zwischen elterlicher und kindlicher Auffälligkeit bestand unabhängig von gleichzeitig vorhandenen Auffälligkeiten in der Familie. Als Mediator zwischen elterlichen und kindlichen Auffälligkeiten erwiesen sich frühe Störungen der Mutter-Kind-Beziehung. Nicht allein die psychische Auffälligkeit der Mutter beeinflußte die frühkindliche Entwicklung, mütterliche Erfahrungen im Umgang mit ihrem Kind wirkten sich umgekehrt auch auf ihre Gesundheit aus.
