Zusammenfassung

Der hypoplastische Daumen gehört zu den rückläufigen Fehlbildungen der menschlichen Hand und tritt in vielfältigen Formen in Erscheinung. Es werden je nach dem Umfang der knöchernen Reduktion fünf Schweregrade unterschieden, die sich in einer teratologischen Reihe ordnen lassen und von der leichten Verkürzung und Verschmächtigung des 1. Strahles bis zur totalen Aplasie reichen. In morphologischer Hinsicht bestimmen außerdem Hypoplasien und Aplasien der radialen Handwurzelknochen, der Daumenballenmuskulatur und des Bandapparates, Insertionsanomalien der Muskulatur sowie Störungen im Verlauf der Gefäße und Nerven das Bild der Deformität. Nur die Hypoplasien ersten Grades führen zu geringen, praktisch kaum ins Gewicht fallenden Funktionsstörungen; alle anderen Formen mindern die Gebrauchsfähigkeit des Daumens erheblich, manche heben sie vollständig auf. Nur auf operativem Wege kann eine Verbesserung dieses auch kosmetisch unbefriedigenden Zustandes herbeigeführt werden. Wichtige Voraussetzungen sind entsprechende handchirurgische Erfahrungen, spezielle postoperative Behandlungsmöglichkeiten und ein ausreichender Intelligenzgrad der Patienten. Als bestes Operationsalter wird das 4.–5. Lebensjahr angesehen. Die Indikation zur Operation soll in Zweifelsfällen erst nach ausführlichen Funktionsanalysen der fehlgebildeten Hand gestellt werden.

Die Methoden im einzelnen hängen von der Schwere der Mißbildung ab. Während bei den Hypoplasien ersten Grades nur ausnahmsweise Opponensersatz-Operationen und Hautplastiken zur Vertiefung und Erweiterung der Commissur angezeigt sind, kommt man mit diesen Verfahren allein schon bei Hypoplasien zweiten Grades nicht mehr aus. Hier muß zusätzlich der 2. Mittelhandknochen aus der Adduktionsstellung in Gebrauchsstellung übergeführt und die Instabilität im Daumengrundgelenk beseitigt werden. Bei ausgedehnten Aplasien der Daumenballenmuskulatur dürfte die Verblockung des 1. Metacarpale mit einem Knochenspan zweckmäßiger als mobilisierende Maßnahmen sein. Dritt-, viert- und fünftgradige Daumenfehlbildungen eignen sich im allgemeinen zur Pollizisation des Zeigefingers. Die Technik des Eingriffes wird beschrieben. Lehnen die Angehörigen die Zeigefingerumstellung ab und besitzt der Restdaumen eine brauchbare Größe, so kann ein daumenerhaltender Eingriff versucht werden. Ob die näher ausgeführte Abspaltung der radialen Hälfte des 2. Mittelhandknochens als Ersatz für den fehlenden 1. Mittelhandknochen eine brauchbare Lösung darstellt, bleibt vorerst noch ungewiß. Diese Operation sollte wegen der zu erwartenden Differenzen im Längenwachstum des Transplantates nicht zu früh vorgenommen werden.

Summary

The hypoplastic thumb belongs to the regressive malformations of the human hand and appears in many varieties. Depending on the extent of the reduction of the bone, five degrees are differentiated which can be put into a teratological scheme and range from a minor shortening and thinnes to a total aplasia of the first metacarpus and digit. From the morphological point of view hypoplasias of the radial carpal bones, the thenar muscles and the ligaments, abnormalities of muscle insertions as well as derangements in the course of vessels and nerves additionally determine the picture of the deformity.

Only first degree hypoplasias lead to minor, not really important impairments of function; all other degrees decrease the employment of the thumb heavily, some block it completely. This state which is also not desirable from the cosmetic point of view, can be improved only by an operation. Important prior conditions are an adequate handsurgical training, a special postoperative treatment and a sufficient intelligence quotient of the patient. The fourth and fifth year of age are considered to be optimal for the time of operation. In cases were the indication for an operation is doubtful a thorough analysis of the function of the defective hand should be made first.

The type of operation depends on the degree of the deformity. While operations for the substitution of the m. opponens and skintransplantations for the deepening and widening of the commissure are indicated in first degree hypoplasias only in some cases, these methods, if employed solely, are insufficient in second degree hypoplasias. In these cases, the second metacarpal bone additionally has to be brought from the position of adduction to the position of employment and the instability of the metacarpophalangeal joint has to be eliminated. In case of extensive aplasias of the thenar muscles it is probably more reasonable to fix the first metacarpal bone instead of mobilizing it. Third, fourth and fifth degree deformities of the thumb are generally suitable for a pollicisation of the index finger. The details of this operation are described. If the transpositione of the index finger is not permitted by relatives and the rudimental thumb has a reasonable size, an operation preserving the thumb can be tried. It is still uncertain whether the splitting off of the radial half of the second metacarpal bone, as described here in detail, is a useful solution. This operation should not be performed at an early age in order to avoid expectable differences in the growth of the graft.

Le pouce hypoplasique appartient aux difformités récurrentes de la main humaine et se manifeste sous des formes diverses. On distingue, selon le volume de la réduction osseuse, cinq degrés de gravité qui se laissent classer dans une série tératologique, s'étendant du raccourcissement et affaiblissement médiocre du pouce et du 1er métacarpe inclus jusqu'à l'aplasie totale. Les hypoplasies et les aplasies des os radiaux du carpe, les muscles de l'éminence thénarienne et de l'appareil ligamenteux, les anomalies d'insertion du système musculaire ainsi que les perturbations dans le cours des vaisseaux et des nerfs déterminent, d'autre part, l'aspect de la difformité du point de vue morphologique. Seules les hypoplasies de 1er degré entraînent de faibles perturbations fonctionnelles sans aucun poids; toutes les autres formes réduisent sensiblement la capacité fonctionelle du pouce, quelques unes la supprime complètement. Uniquement par la voie opérative il est possible d'atteindre une amélioration de cet état également insatisfaisant du côté esthétique. De correspondantes expériences chirurgicales de la main, des possibilités spéciales et postopératives de traitement ainsi qu'un suffisant degré d'intelligence du patient en représentent les conditions importantes. La 4ème et 5ème année de vie est considérée comme l'âge d'opération le plus favorable. En cas de doute, l'indication d'opération n'est donnée qu'après avoir effectué des analyses détaillées du fonctionnement de la main malformée. Les méthodes, en particulier, dépendent de la gravité de la malformation. Pendant que les remplacements plastiques du muscle opposant au niveau de la loge hypothénarienne (Opponensersatz-Operationen) et les greffes cutanées sont seulement indiqués exceptionnellement quand il s'agit d'hypoplasies de 1er degré pour effectuer un approfondissement et un élargissement de la commissure, ce procédé s'avère déjà insuffisant en cas d'hypoplasies de 2ème degré. Il faut alors conduire supplémentairement le 2ème os métacarpien de la position d'adduction à une position de fonctionnement et supprimer l'instabilité de l'articulation métacarpo-phalangienne au niveau du pouce.

En cas de grave aplasie du système musculaire de l'éminence thénarienne la stabilisation de la greffe osseuse du 1er métacarpe semble être plus favorable que des mesures de mobilisation. Les difformités de 3ème, 4ème et 5ème degré se prêtent en général à la pollicisation de l'index. La technique a été décrite. Si les parents s'opposent à la pollicisation de l'index et si le pouce résiduel a une longueur utilisable, on peut essayer une intervention susceptible de maintenir le pouce. Si le dédoublement, expliqué en détail, de la moitié radiale du 2ème os métacarpien en remplacement du 1er os métacarpien faisant défaut, représente une solution favorable, reste préalablement incertain. Cette opération ne devrait être effectuée trop tôt en raison des différences probables dans la croissance longiligne du greffon.
