Samenvatting

Dit hoofdstuk begint met een uiteenzetting over de myofasciale bindweefselplaatsystemen in en rond de schouder en de belangrijke rol van het losmazig reticulaire MCDAS. Naast het glenohumerale systeem (HSMI) is er ruime aandacht voor het primaire en secundaire scapulothoracale systeem. Na de beschrijving van de descriptieve anatomie en de osteo- en artrokinematica komen veelvoorkomende aandoeningen als schouderinstabiliteit, het SubAcromiale PijnSyndroom (SAPS) en de frozen shoulder (inclusief de SNN-praktijkrichtlijn frozen shoulder 2017) aan bod. De rode vlaggen worden besproken en er wordt uitgebreid stilgestaan bij aspecifieke schouderpijn (‘medisch onbegrepen’ pijn). In het kader van het lichamelijk onderzoek staan nu – in plaats van provocatietests – de reductietests centraal, zoals de Combined Reduction Test (of: ‘Manus-van-alles’-techniek), de gedupliceerde circumductie- en deviatiebeweging, SAT, SRT, GHRT en de SSMP. De bekende provocatietests met hoge sensitiviteit hebben nog slechts excluderende waarde bij een negatieve uitkomst. Tot slot presenteren de auteurs als schouderexperts – naast een handige ‘patronenmatrix’ – het fundament voor onderzoek- en behandelprotocollen bij aspecifieke en mild specifieke schouderpijn. Er worden 78 onderzoeks- en behandeltechnieken van de schoudergordel uitgebreid beschreven en op video gedemonstreerd. De thoracale, cervicothoracale én cervicale wervelkolomtechnieken komen uitgebreid aan bod in de H. 10, 11 en 12.

anteriorrotatie
De osteokinematische spinbeweging van een botstuk (scapula, clavicula) om een frontotransversale as (vooroverkanteling).
aspecifieke bewegingstechniektechniek
Bewegingstechniek waarbij alle kapseldelen gelijkmatig worden belast.
Voorbeeld: een tractietechniek van het caput humeri – vanuit de MLPP – in laterale, iets ventrale en iets craniale richting ten opzichte van de cavitas glenoidalis. Dit wordt ook een distractie genoemd: een tractie vanuit de ruststand in de richting van de normaal.
bindweefselplaten
Dit zijn myofasciale systemen, zoals spierfascies, kapsels, ligamenten, pezen en aponeurosen (peesplaten). Bindweefselplaten kunnen de vorm hebben van schotten, enveloppen en kokers, zoals de buitenste isolatie rond elektrakabelsystemen en de isolatie daarbinnen rond de individuele elektradraden. Alle myofasciale bindweefselplaten vormen onderling een continuïteit met elkaar en met het totale benige skelet.
cocking position
De derde fase in de werpbeweging, waarbij de schouder zich naar de MCPP begeeft en de 90° geflecteerde elleboog in valgusstress komt. Fase 1: de ‘wind-up’ (de opwarm-‘krul’). Fase 2: de ‘early cocking’ (bal en hand gaan naar achter). Fase 3: de ‘late cocking’ (waarbij de bal naar zijn verste punt gaat door de exorotatie van de schouder tijdens de achterwaartse beweging van de arm). Fase 4: de ‘acceleration’ (waarin na de versnelling de bal uiteindelijk de hand verlaat).
directe techniek (aangrijpingspunt)
Het aangrijpingspunt van de techniek ligt op het botstuk dat mede het gewricht vormt (zie ook ‘primaire techniek’).
extern schouderimpingement
Extra-articulair glenohumeraal impingement. Er treedt compressie op buiten het glenohumerale gewricht (Burkhart et al. 2003; Koel 2008).
fuzz
De anatoom Gil Hedley gebruikt het woord ‘fuzz’ voor verstijfd losmazig reticulair bindweefsel (MCDAS), dat zich als glijmiddel tussen de weefsels en organen bevindt. MCDAS verstijft wanneer er geen bewegingsprikkels op los worden gelaten. Bewegen noemt hij ‘melting the fuzz’. Hij vergelijkt het MCDAS met stijve brillantine die vloeibaar wordt wanneer je het tussen je handen wrijft. Het MCDAS draagt ertoe bij dat bindweefselplaten ten opzichte van elkaar kunnen verschuiven.
indirecte techniek (aangrijpingspunt)
Het aangrijpingspunt van de techniek ligt op een ander botstuk dan de botstukken die het te bewegen gewricht vormen (zie ook ‘secundaire techniek’).
Humero Scapular Motion Interface (HSMI)
De totale ‘koppeling’ tussen humerus en scapula. Het caput humeri articuleert niet alleen met de cavitas glenoidalis van de scapula, maar ook indirect met het acromion, het processus coracoideus en het ligamentum coracoacromiale. Het nauwsluitende contact en het centreren van de humeruskop in de kom wordt bewerkstelligd door – van diep naar oppervlakkig – de rotatorenmanchet met de rotatorcable, de bursa, de m. deltoideus, de aan het processus coracoideus bevestigde spieren en zelfs de lichaamsfascie, het onderhuidse vet- en bindweefsel en de huid. Zo ontstaat een hecht koppelingssysteem van een gesmeerde kogel in een diepe en flexibele kom, dat een grote ROM koppelt aan voldoende stabiliteit. De HSMI is geschikt voor bewegingen waarbij zowel kracht, lenigheid als snelheid zijn vereist, zoals ballistische werpbewegingen.
incidentie
Het aantal (nieuwe) gevallen met de aandoening die zich in een populatie voordoen gedurende een bepaalde periode.
intern schouderimpingement
Intra-articulair impingement. Er treedt compressie op binnen het glenohumerale gewricht (Burkhart et al. 2003; Koel 2008).
Kappa-waarde
Een maat voor intra- en interbeoordelaarsbetrouwbaarheid. Een kappa van 0 betekent dat de overeenkomst tussen beoordelingen volledig op kans berust, een kappa van 1 is een volledige overeenkomst.
posteriorrotatie
De osteokinematische spinbeweging van een botstuk (scapula, clavicula) om een frontotransversale as (achteroverkanteling).
prevalentie
Het aantal personen met een bepaalde aandoening op een bepaald tijdstip in een populatie.
provocatietest
Een diagnostisch onderzoekmiddel (een zogenaamde ‘special test’) waarmeen getracht wordt een structurele beschadiging of ontsteking te lokaliseren en indien mogelijk de beschadigde structuur te benoemen. De gedachte was dat door primaire hyperalgesie of perifere sensitisatie de structuur meer nocisensoriek dan gewoonlijk zou gaan produceren. De laatste jaren is echter evident geworden, dat ‘special tests’ niet in staat zijn een beschadigde of ontstoken structuur te lokaliseren. Zeker niet indien er sprake is van centrale sensitisatie. Provocatietests met een hoge sensitiviteit hebben nog slechts excluderende waarde indien het testresultaat negatief is.
reductietest
Een diagnostisch onderzoekmiddel waarmee getracht wordt de symptomen die de patiënt vertoont, bijvoorbeeld pijn, te verminderen (reduceren). Door het gewricht tijdens de provocerende beweging goed te centreren kan de pijn verminderen en zelfs verdwijnen. Hiervoor zijn manuele vaardigheden zoals de Combined Reduction Test (CRT) of de Scapula Assistance Test (SAT) zeer geschikt.
rotatorcable
De gemeenschappelijke aanhechting van de rotatorcuffspieren (m. supraspinatus, m. infraspinatus, m. subscapularis en m. teres minor) op het caput humeri. De rotatorcable heeft dankzij zijn rond de humeruskop geformeerde, myofasciale ‘envelop’-structuur samen met de m. teres major (de ‘vijfde cuffspier’) ook een grote invloed op de glenohumerale stabiliteit (het ‘centreren’).
rotatoreninterval
Een niet-contractiel deel van de rotatorcuff en rotatorcable dat zich bevindt tussen de m. supraspinatus en de m. subscapularis en recht boven het intra-articulaire deel van de lange kop van de m. biceps brachii. Op de plaats van de rotatoreninterval bevindt zich het lig. coracohumerale.
scaptie
Abductie van de humerus in het schuine vlak van de scapula, precies midden tussen het sagittale en frontale vlak. Er wordt bewogen in de richting van de MLPP: 60° abductie, 60° anteflexie, iets exorotatie.
Stanmore Triangle
De gelijkzijdige driehoek staat symbool voor de drie (extreme) types van schouderinstabiliteit op de hoekpunten. De combinatievormen bevinden zich binnen de driehoek. Operatief ingrijpen hoort bij de top van de driehoek – Type I: traumatische instabiliteit, met structurele schade, zoals de TUBS). Conservatieve aanpak hoort thuis vooral in de basis van de driehoek. De rechter basishoek – Type II: zowel traumatische als atraumatische structurele instabiliteit, zoals de hyperlaxiteit bij het AIOS-type (microtraumata), de verworven instabiliteit. De linker basishoek – Type III: atraumatische, niet-structurele instabiliteit (de ‘wobbly shoulder’) zoals het AMBRII type.

In de handpalm, de voetzool en de hals (platysma) zijn de huid en de onderhuidse lichaamsfascie met elkaar verbonden. Huid en lichaamsfascie kunnen daar dus niet ten opzichte van elkaar bewegen vanwege het ontbreken van tussenliggend MCDAS.

Een XCO-trainer is een aluminiumbuis, voor tweederde gevuld met een scherfvormige minerale delfstof, waarmee ook eindstandig en toch met lage intensiteit driedimensionaal in de vrije ruimte kan worden getraind. Het in 2003 op de markt gekomen trainingshulpmiddel kreeg in 2005 de naam XCO-trainer. Het is uitgevonden door collega-manueeltherapeut Jan Hermans (Rotterdam), die er in 2003 de Innovation Award voor ontving. Vooral het driedimensionale bindweefselskelet rondom de geïsoleerde spiervezel (endomysium) kan veilig en effectief worden belast.
