10. Septische artritis

Inleiding

Septische artritis ofwel infectieuze artritis is een gewrichtsontsteking die wordt veroorzaakt door een micro-organisme in het gewricht, meestal een bacterie, en vormt een spoedindicatie van antibiotische behandeling om gewrichtsdestructie te voorkomen. Dit in tegenstelling tot reactieve artritis, die ontstaat als reactie op een bacteriële infectie elders in het lichaam, meestal van de tractus urogenitalis of digestivus. Antistoffen gericht tegen de bacterie kunnen een kruisreactie aangaan met het synovium, waardoor een passagère, steriele ontsteking ontstaat. De reactieve artritis wordt meestal gezien in het kader van de spondyloartritis (H. 8) en behoeft geen antibiotische behandeling. ‘Acuut reuma’ is een vorm van reactieve artritis die wordt veroorzaakt door een groep-A-streptokok. Deze ziekte komt nog maar zelden in Nederland voor.
