4a Addendum: het trochanter major pijnsyndroom

Samenvatting

De trochanter major is een grote apofyse waar belangrijke spieren aanhechten. Deze spieren zorgen voor het bewegen van het been tijdens de zwaaifase en hebben een belangrijke functie bij het ‘dragen’ van het lichaam op het standbeen; vrijwel het gehele lichaam wordt gedragen door de heupkop terwijl de heupabductoren voorkomen dat men door de heup zakt.

De term ‘trochanter major pijnsyndroom’ is overgenomen uit de Engelse literatuur: The greater trochanteric pain syndrome. Deze term wordt in dit artikel gebruikt omdat er veel verwarring bestaat over de etiologie van lokale laterale heuppijn. Meestal wordt aan patiënten met dit type klachten ten onrechte de diagnose ‘bursitis trochanterica’ gegeven.

Een per exclusionem gestelde diagnose is een diagnose die gesteld wordt door het uitsluiten van andere mogelijkheden.

In het geval van chronische achillespeesklachten wordt zwelling van de pees gewoonlijk wel opgemerkt omdat deze zich oppervlakkig onder de huid bevindt. Zwelling wordt bij tendinose veroorzaakt door een toename van de hoeveelheid matrix tussen de collagene vezels binnen de pees.

Positieve Trendelenburg: door zwakte of pijn van de heupabductoren ontstaat een waggelend gangpatroon waarbij het bekken doorzakt aan de zijde die niet meer wordt ondersteund.

Positieve Duchenne: door zwakte of pijn van de heupabductoren ontstaat een gangpatroon waarbij het lichaam steeds overhelt naar het aangedane standbeen.
