Lifelong premature ejaculatie: een neurobiologische hypothese over erectio praecox, hypererotisch toestandsbeeld en detumescentia praecox

Samenvatting

Op basis van farmacologisch in vivo dieronderzoek, en humaan klinisch en genetisch onderzoek is een neurobiologische visie ontwikkeld op lifelong premature ejaculatie (PE). Een aantal symptomen van lifelong PE wordt echter nog steeds niet begrepen. In dit artikel wordt beargumenteerd dat lifelong PE niet alleen gekenmerkt wordt door vroegtijdige ejaculaties (ejaculatio praecox) maar ook door vroegtijdige erecties (erectio praecox) en een na de zaadlozing snel optredende verslapping van de penis (detumescentia praecox) als onderdeel van een (sub)acuut optredend hypertoon of hypererotisch fysiek toestandsbeeld bij erotische of seksuele activiteit. Op basis van dieronderzoek wordt gepostuleerd dat de snelle zaadlozing, snelle erectie en snelle detumescentie geassocieerd zijn met centraal en perifeer verhoogde oxytocine-release. Onderzoek naar deze drie kenmerken van het (sub)acuut optredend hypererotische toestandsbeeld draagt vermoedelijk bij aan een beter fenomenologische beschrijving van en beter neurobiologisch inzicht in lifelong PE ten opzichte van de drie andere PE-subtypen.
