3. Licht traumatisch hoofd-/hersenletsel

Samenvatting

Licht traumatisch hoofd-/hersenletsel (THL) is een van de meest frequente neurologische aandoeningen in Nederland. In de acute fase is een belangrijke rol weggelegd voor de CT-scan om de aanwezigheid van traumatische (intra-)craniële afwijkingen aan te tonen. Zeker één op de vier patiënten met licht THL heeft langdurig last van posttraumatische klachten die onder meer het hervatten van werk of studie en andere dagelijkse activiteiten negatief beïnvloeden. Patiënten met een verhoogde kans op een ongunstige uitkomst dienen tijdig te worden geïdentificeerd, zodat zij adequaat kunnen worden begeleid. Enkele factoren geassocieerd met een suboptimaal functioneel herstel zijn: de ernst van het letsel, leeftijd, stemmingsklachten, opleidingsniveau, copingstijl, medische voorgeschiedenis en vroege posttraumatische klachten zoals nekpijn. Hierop zijn diverse prognostische modellen gebaseerd. In de chronische fase kan MRI-onderzoek van de hersenen en neuropsychologisch onderzoek worden overwogen. Een belangrijke rol in de behandeling van licht THL is weggelegd voor psycho-educatie.
