Samenvatting

Achtergrond

Insulineresistentie is een belangrijke factor in de pathogenese van diabetes mellitus type 2. In pasgeborenen kunnen prematuriteit of een laag geboortegewicht (small for gestational age, SGA) bijdragen aan de ontwikkeling van een verminderde insulinegevoeligheid. Doel van deze studie was het meten van glucose- en insulineconcentraties en het berekenen van HOMA (Homeostasis Model Assessment) om de insulinesensitiviteit te evalueren bij zuigelingen met verschillende zwangerschapsduur en geboortegewicht. Het moment van bloedafname werd op de à terme datum gesteld om de groepen vergelijkbaar te houden wat betreft maturatieniveau.

Methoden

Cross-sectionele observationele pilotstudie. Pasgeborenen na een zwangerschapsduur tussen 25 en 42 weken zonder belangrijke comorbiditeit met meer dan 120 ml/kg/dag enterale voeding rond de à terme datum werden geïncludeerd. Bloedmonsters voor glucose en insuline werden 3 uur na de laatste voeding afgenomen.

Resultaten

In 35 van de 73 neonaten (48%) was de insulineconcentratie onder de detectielimiet van de assay. Er waren geen statistisch significante verschillen in de patiëntenkarakteristieken van neonaten met een insulineconcentratie boven versus onder de detectielimiet. Echter, de glucoseconcentratie was hoger in de kinderen met een insulineconcentratie boven de detectielimiet.

Conclusie

Wij vonden geen verschillen tussen zwangerschapstermijn- of geboortegewichtgroepen in de parameters voor beoordeling van insulinesensitiviteit op de à terme leeftijd. Insulineconcentratie was in bijna de helft van onze studiegroep onder de detectielimiet, suggestief voor een goede insulinegevoeligheid op de à terme leeftijd. Echter, hierdoor is een simpel homeostasemodel zoals HOMA ongeschikt voor onderzoek naar insulinegevoeligheid in deze patiëntenpopulatie.
