6. Kwaliteitsverbetering in de dagelijkse praktijk

Samenvatting

Waardevolle inzichten over gewenste zorg vinden meestal niet vanzelf hun weg naar de dagelijkse praktijk. Hierdoor wordt, veelal onbewust, optimale zorg onthouden aan patiënten. In dit hoofdstuk wordt een systematische aanpak uiteengezet om tot kwaliteitsverbetering in de dagelijkse zorgpraktijk te komen. Het startpunt van kwaliteitsverbetering in de gezondheidszorg kunnen de ideeën of ervaringen zijn binnen de eigen werksetting over hoe de patiëntenzorg beter kan. Daarnaast kan er buiten de eigen werksetting nieuwe wetenschappelijke informatie beschikbaar komen die erop wijst dat de zorg voor patiënten beter kan. Beide aanleidingen kunnen een proces van verandering, ofwel kwaliteitsverbetering, in gang zetten. Om tot succesvolle kwaliteitsverbetering te komen, is het belangrijk om stapsgewijs op geplande en systematische wijze verbeteringen door te voeren. Allereerst dient helder geformuleerd te worden wat ‘goede zorg’ precies inhoudt, zodat gemeten kan worden of deze zorg ook daadwerkelijk toegepast wordt in de dagelijkse praktijk (stap 1). Vervolgens dient in kaart gebracht te worden waarom de huidige zorg eigenlijk is zoals die is en welke factoren van invloed zijn op het wel of niet toepassen van de ‘goede’ oftewel gewenste zorg (stap 2). Op geleide van deze informatie dient een passende verbeterstrategie gekozen te worden (stap 3), die vervolgens uitgevoerd en geëvalueerd wordt (stap 4). Deze aanpak van kwaliteitsverbetering – oftewel het implementeren van gewenste zorg – wordt uitgebreid beschreven in het boek Implementatie: effectieve verbetering van de patiëntenzorg van Wensing en Grol (2017).
