Kwaliteit van spirometrie in de eerste lijn

Achtergrond

Spirometrie is onmisbaar bij de diagnostiek en controle van chronische luchtwegaandoeningen. Ruim de helft van de Nederlandse huisartsenpraktijken doet spirometrietests in eigen beheer, maar ze kunnen hun patiënten ook verwijzen naar een huisartsenlaboratorium. Wij gingen na hoe de kwaliteit van uitvoering van spirometrietests in huisartsenlaboratoria zich verhoudt tot die in huisartsenpraktijken.

Methoden

Uit de gezamenlijke spirometriebestanden van 10 huisartsenpraktijken en het bestand van 1 huisartsenlaboratorium trokken wij 2 aselecte steekproeven van elk 141 prebronchodilatoire spirometrietests. Onafhankelijke, ervaren longfunctielaboranten beoordeelden de geanonimiseerde tests op kwaliteit (volgens internationale kwaliteitscriteria) en klinische bruikbaarheid (op grond van hun eigen expertise).

Resultaten

Van de spirometrietests in het huisartsenlaboratorium voldeed 60,3% aan de internationale kwaliteitscriteria; in de huisartsenpraktijken was dat 31,9% (odds ratio (OR) 3,21; p < 0,001). De longfunctielaboranten beoordeelden 96,5% van de tests in het huisartsenlaboratorium als klinisch bruikbaar, versus 83,7% van die in de huisartsenpraktijken (OR 5,13; p < 0,001).

Conclusie

Spirometrietests in huisartsenlaboratoria voldeden veel vaker aan de strikte internationale kwaliteitscriteria dan de tests die huisartsenpraktijken in eigen beheer uitvoeren. Onze beoordelaars achtten een aanzienlijk deel van de tests die kwalitatief tekortschoten echter wel klinisch bruikbaar. Vooralsnog blijft onduidelijk wat de eventuele consequenties voor de patiënt zijn van een spirometrietest die niet aan de internationale normen voldoet.
