Samenvatting

Voor pijn bij artritis en acute pijn zijn non-steroïd anti-inflammatoire drugs (NSAID) zeer geschikt gebleken. NSAID’s werken door middel van het remmen van cyclo-oxygenase (COX), ofwel prostaglandinesynthese. Hierdoor wordt de transformatie van arachidonzuur naar prostaglandines, prostacyclines en tromboxaan geremd. Er zijn 2 isovormen bekend. COX-1 wordt voortdurend in de weefsels aangemaakt, vooral in het maag/darmslijmvlies. Dit is de verklaring voor het verhoogde gastro-intestinale bloedingsrisico, dat de grootste beperking vormt van (langdurig) NSAID-gebruik. COX-2-expressie kan worden geïnduceerd, vooral door ontsteking. COX-2 is ook een promotor van intestinale tumorgenese gebleken.
