Kostenanalyse van de diagnostiek bij verdenking op urolithiasis

Samenvatting

Doel: Analyseren van de kosten van de huidige standaarddiagnostiek bij verdenking op urolithiasis.
Methode: Alle 390 geopende DBC’s met betrekking tot kolieken van mei 2010 tot mei 2011 zijn geanalyseerd, 217 patiënten voldeden aan de inclusiecriteria (anamnestisch koliekpijnen, geen zwangerschap, gegevens in dit ziekenhuis bekend). De resultaten van verrichte beeldvorming zijn genoteerd. Vervolgens zijn de werkelijk gemaakte kosten voor beeldvorming vergeleken met de kosten die gemaakt zouden zijn indien alle patiënten alleen een CT-scan hadden ondergaan.
Resultaten: Er zijn 207 echo’s, 185 buikoverzichtsfoto’s en 112 CT’s verricht. Kosten voor een buikoverzichtsfoto (X-BOZ), echografie van de nieren en CT-abdomen waren resp. € 41,34, € 102,77 en € 168,33. De werkelijk gemaakte kosten zijn € 47.774; de berekende kosten voor 217 CT’s zijn € 36.527.
Conclusie: Vanuit kostenaspect kan worden overwogen bij alle patiënten met verdenking op urinewegstenen primair een blanco CT-scan van de urinewegen te maken. Een en ander dient te worden afgewogen tegen de risico’s van de hogere stralenbelasting.
