Rolmodellen en beloningen ter verbetering van therapietrouw bij jonge verslaafden

Samenvatting

Geringe therapietrouw betekent een beperking van de effectiviteit van ambulante verslavingszorg. De uitval uit de behandeling is in het bijzonder hoog onder jongeren, wat resulteert in hoge terugvalpercentages en een matige tot slechte prognose wat betreft psychische, fysieke en sociale gezondheid. Interventies ter verbetering van de behandeltrouw kunnen bijdragen aan vermindering van de persoonlijke en maatschappelijke belasting door verslaving bij jongeren. ‘Evidence-based’ methoden om dit te doen ontbreken echter. Voor dit onderzoek werden 32 verslaafde jongeren at random toegewezen aan een standaardbehandeling (controlegroep) of standaardbehandeling aangevuld met motiverende sessies door rolmodellen, zoals bekende professionele voetballers (interventiegroep), met als doel de drop-out te verminderen. Bij follow-up na zes maanden was het uitvalpercentage in de controlegroep 44 %, vergeleken met 6 % in de interventiegroep. Daarnaast lieten alleen de jongeren in de interventiegroep significante afname zien in zelfgerapporteerde trek en psychische stress, en een verbetering in kwaliteit van leven op verschillende leefgebieden. Onze conclusie is dat de gecombineerde inzet van rolmodellen en beloningen in de behandeling een positief effect kan hebben op behandeltrouw en -uitkomst bij jonge verslaafden. Verder onderzoek is nodig om deze bevindingen te bevestigen en te generaliseren naar andere patiëntenpopulaties en andere typen rolmodellen.
