37 Katheterablatie van hartritmestoornissen

Samenvatting

De eerste beschrijving van een katheterablatie dateert uit 1979: tijdens externe defibrillatie bij een diagnostisch elektrofysiologisch onderzoek ontstond een derdegraads AV-blok door accidentele ablatie van de hisbundel, vermoedelijk als gevolg van kortsluiting in het systeem. Aansluitend aan deze complicatie werden vanaf 1982 de eerste artikelen gepubliceerd over bewuste ablaties van de hisbundel of van een extra verbinding bij het wolff-parkinson-whitesyndroom (WPW-syndroom), waarbij men gebruikmaakte van directestroom- (DC-)schokken via een endocardiale katheter. De elektrofysiologische interventie heeft in de afgelopen 25 jaar een enorme vlucht genomen. DC-ablatie werd beperkt door verschillende nadelen, zoals de noodzaak tot algehele anesthesie en gevaar van perforatie van het hart, beide een gevolg van de plotselinge drukverhoging met een DC-shock via een katheter met een kleine tip (en dus lokaal een zeer hoge energiedichtheid). Veel van deze beperkingen werden opgelost met de komst van de radiofrequente (RF-)ablatie eind jaren tachtig. Hierbij wordt een hoogfrequente wisselstroom afgegeven tussen de tip van een ablatiekatheter en een rugplaat (feitelijk niet anders dan bij diathermie zoals al veel langer gebruikt door chirurgen), waarmee weefsel lokaal wordt verhit. Deze techniek bleek beter controleerbaar en minder traumatisch dan DC-ablatie en daarmee werd – zonder narcose – ablatie van een groeiend aantal ritmestoornissen mogelijk. Door de stuurbaarheid van de ablatie en doordat de temperatuur geleidelijk kon worden opgevoerd, konden ook structuren dicht bij de AVknoop voor het eerst worden geableerd. Daarmee kreeg men in de jaren negentig de mogelijkheid AV-nodale re-entrytachycardieën (AVNRT) te genezen, naast atriumfibrilleren de meest voorkomende supraventriculaire tachycardie (SVT).
