Samenvatting

Jicht is een extreem pijnlijke paroxismale inflammatoire artritis, meestal met een plotseling en dramatisch begin, niet zelden juist ’s nachts. Het heeft dikwijls een typische voorkeurslocatie (het MTP1-gewricht), wordt gekenmerkt door een relatief korte aanvalsduur (dagen tot 2-3 weken) en recidiveert vaak. Een huisarts met een normpraktijk heeft jaarlijks te maken met tien tot twaalf jichtpatiënten, onder wie twee tot vier nieuwe gevallen. Hoewel er een gouden standaard voor de diagnose bestaat (microscopisch aantoonbare uraatkristallen in geaspireerde gewrichtsvloeistof) stelt de huisarts in bijna alle gevallen jicht vast alleen op basis van het klinische beeld. De acute aanvalsbehandeling bestaat uit een NSAID, colchicine of kortdurend een corticosteroïd. Soms is urinezuurverlagende medicatie nodig. Jichtpatiënten blijken cardiovasculaire risicopatiënten te zijn. Bijna de helft van hen heeft bijvoorbeeld hoge bloeddruk. Een jichtpatiënt verdient aandacht in het kader van eerstelijns cardiovasculair risicomanagement, zoals vastgelegd in de richtlijn hierover van het Nederlands Huisartsen Genootschap.
