Infant Mental Health in het ziekenhuis

Psychotherapeutisch werk op afdelingen verloskunde en neonatologie

Samenvatting

Een gehechtheidsrelatie tussen ouders en hun kind begint tijdens de zwangerschap en wordt verder gevormd in contact met het kind. In dit artikel wordt beschreven wat (aanstaande) ouders bij belaste zwangerschappen en extreme vroeggeboorte van hun kind meemaken in een academisch ziekenhuis. Hun zorgen, dilemma’s en worstelingen kunnen gevolgen hebben voor hun functioneren als ouders, de relatie met hun kind en met de staf van het ziekenhuis. De ervaringen en de mogelijke gevolgen voor de ouder-kindrelatie worden vanuit een Infant Mental Health-visie beschreven en aan de hand van een kwalitatief en inventariserend dossieronderzoek nader uiteengezet. In 2013 kregen 61 ouderparen psychotherapeutische ondersteuning. Gespreksthema’s waren onder andere moeite hebben met de aanblik van het eigen kind en moeite in de communicatie met professionals. De psychologen deden interventies gericht op ondersteuning van de ziekenhuisopname, de ouder-kindrelatie en het ondersteunen van het verpleegkundig en medisch personeel. Er werden hierbij onder andere mentaliseren bevorderende technieken gebruikt. De therapeutische attitude bestond onder meer uit het creëren van een holding environment en het bieden van containment. De psychologen probeerden splijting en fragmentatie tegen te gaan, door communicatie te faciliteren tussen professionals en tussen ouders en professionals. Psychotherapeutische hulp op klinische afdelingen verloskunde en neonatologie blijkt in een behoefte te voorzien.
