Implementatie van antitrombotische behandeling bij atriumfibrilleren

DOEL Atriumfibrilleren kent een hoge prevalentie. Een belangrijk deel van de behandeling bestaat uit het voorkómen van een beroerte. De NHG-Standaard, gebruikmakend van de chads2-score, is duidelijk ten aanzien van het maken van een keuze voor een antitrombotisch medicament. Over- en onderbehandeling met orale antistolling lijken beide echter nog veel voor te komen. Door middel van dit implementatieonderzoek wilden we deze onderen overbehandeling binnen onze praktijk kwantificeren én corrigeren. We keken ook naar de tijdsinvestering die voor het hele project nodig was.
METHODE Wij onderzochten binnen een stadspraktijk van 2600 patiënten alle mensen met atriumfibrilleren (n = 80) op chads2- score en actuele antitrombotische medicatie. Zo nodig pasten we deze medicatie aan.
RESULTATEN Deze analyse leidde tot 13 medicatiewijzigingen. Zesmaal gingen we over op orale antistolling, zesmaal op acetylsalicylzuur. Eén patiënt startte met acetylsalicylzuur. Deze richtlijnimplementatie bij 80 patiënten nam voor de huisarts 17,5 uur in beslag.
CONCLUSIE Actief implementeren van de richtlijn ten aanzien van antitrombotische therapie bij atriumfibrilleren is medisch zinvol en kan waarschijnlijk beroertes voorkomen. Men moet wel werktijd investeren om deze kwaliteitsverbetering te realiseren. Een vergoeding voor dit soort werk lijkt een discussie waard.

AIM The prevalence of atrial fibrillation is high and treatment is focused on stroke prevention. The Dutch College of General Practitioners Practice Guideline, which makes use of the CHADS score, provides clear advice about the need for antithrombotic medication, but undertreatment and overtreatment with oral anticoagulants are still quite common. We examined the extent to which undertreatment and overtreatment occurred in our practice and, where necessary, revised the medication regimen. We also investigated the time needed for this.
METHOD We investigated the CHADS score and use of antithrombotic medications in 80 patients with atrial fibrillation in our urban practice covering 2600 patients.
RESULTS The investigation led to 13 medication changes: a switch to oral anticoagulant in 6 cases, a switch to acetylsalicylic acid in 6 cases, and initiation of acetylsalicylic acid in 1 case. Implementation of the guideline in the 80 patients took 17.5 hours.
CONCLUSIONS Active use of the guideline on antithrombotic treatment in atrial fibrillation is medically justified and could prevent stroke, but this improvement requires an investment of time. It might be worthwhile discussing whether this type of activity should be remunerated.
