7. Impact van neuropsychologische klachten op de behandeling van de posttraumatische stressstoornis

Samenvatting

Patiënten met een posttraumatische stressstoornis (PTSS) vertonen verscheidene lichte tot matige cognitieve problemen. Verschillende hersengebieden en neurobiologische systemen spelen een rol bij deze cognitieve problemen. Zo wordt er bij patiënten met PTSS een verhoogde activiteit van de amygdala en een relatief verminderde activiteit van de mPFC gevonden en is er sprake van disfuncties in het hippocampaal functioneren. Bij patiënten met PTSS is er sprake van een overgevoelig stresssysteem. Verstoringen in de HPA-as en een overgevoelig noradrenerg systeem spelen hierin een belangrijke rol. Cognitieve problemen lijken te verminderen na succesvolle psychotherapeutische behandelingen. Andersom blijkt dat cognitieve problemen voorspellers kunnen zijn voor het behandelsucces bij patiënten met PTSS. In de behandeling van PTSS dient er rekening te worden gehouden met cognitieve problemen van patiënten met PTSS. Aanbevelingen voor behandelaren zijn onder meer het beknopt en via meerdere modaliteiten aanbieden van informatie en toevoegen van reminders van traumata tijdens traumaverwerking, zoals middels virtual reality.
