Hypothalamus-hypofyse-bijnier-asfunctie in overlevers van acute lymfatische leukemie op de kinderleeftijd en in gezonde controles

Samenvatting

Acute lymfatische leukemie (ALL) is de meest voorkomende vorm van kanker op de kinderleeftijd. Glucocorticoïden vormen een belangrijk onderdeel van de behandeling van ALL op de kinderleeftijd. Suprafysiologische doseringen kunnen langdurige onderdrukking van de hypothalamus-hypofyse-bijnieras (HPA-as) veroorzaken. Aan de hand van deze multicenterstudie hebben wij onderzoek gedaan naar de HPA-as-functie bij kinderen die behandeld zijn voor ALL in vergelijking met qua leeftijd en geslacht overeenkomende gezonde controles. De HPA-as-functie werd onderzocht aan de hand van de cortisol awakening response en de lage dosis dexamethasonsuppressietest. Vanwege een mogelijke associatie met cortisolspiegels werd er door middel van vragenlijsten informatie verzameld over slaap, vermoeidheid en kwaliteit van leven. Drieënveertig overlevers van ALL en 57 controles werden in deze studie geïncludeerd. Er was geen verschil tussen beide groepen in de cortisol awakening response, maar overlevers van ALL hadden hogere ochtendcortisolspiegels (16,31 nmol/l (ALL) vs. 13,66 nmol/l (controles), p = 0,013) en meer cortisolsuppressie na de dexamethasonsuppressietest (p < 0,001) in vergelijking met de gezonde controles. Hoge cortisolspiegels waren geassocieerd met meer vermoeidheid en een slechtere fysieke en psychosociale kwaliteit van leven (r –0,46, p = 0,002, r –0,39, p = 0,012 en r –0,33, p = 0,036, respectievelijk). De ervaring van een traumatische gebeurtenis in het verleden, zoals ALL op de kinderleeftijd, kan leiden tot langdurige dysregulatie van de HPA-as, zich uitend in een hogere cortisolproductie en een toegenomen feedback van de HPA-as, en lijkt geassocieerd met klinisch relevante problematiek.
