Hoogteziekte en de huisartsenpraktijk

Samenvatting

Acute hoogteziekte komt op hoogten boven de 2500 meter veelvuldig voor. Hoogtelongoedeem en hoogtehersenoedeem zijn zeldzamer, maar wel levensbedreigend. Snel stijgen boven de 2500 m is de belangrijkste determinant. De preventie van deze ziekten ligt dan ook vooral in een verstandig stijgtempo. Omdat dit niet altijd mogelijk is, kan medicamenteuze ondersteuning zinvol zijn. Vooral het preventief gebruik van acetazolamide bij acute hoogteziekte komt voor toeristen soms in aanmerking. Het middel is bewezen effectief en grijpt causaal aan; het is dus geen symptoombestrijder.

Dit artikel beschrijft tevens het curatief gebruik van medicijnen bij de genoemde ziekten die aan de hoogte toe te schrijven zijn. De huisarts is daar weliswaar niet bij betrokken, maar reisorganisaties dienen op de hoogte te zijn van de juiste aanpak.

Het artikel gaat tot slot kort in op enkele bestaande ziekten die een hoge tocht soms ongewenst maken. Hiertoe worden contra-indicaties vermeld, die overigens vooral op inzicht en empirie berusten; wetenschappelijke evidentie ontbreekt merendeels. In veel gevallen is een persoonlijke evaluatie noodzakelijk.
