15. Hoogtegeneeskunde – fysiologie en pathofysiologie

Samenvatting

In dit hoofdstuk worden de belangrijkste fysiologische veranderingen besproken die optreden wanneer een persoon wordt blootgesteld aan hypoxie ten gevolge van een verblijf op hoogte. Dit zijn onder andere een toename van de ventilatie, een daling van de PCO2 en een toename van het Hb-gehalte. Wanneer deze aanpassingen niet goed verlopen of wanneer de toename in hoogte (hypoxie) te snel gaat, kunnen er hoogtegerelateerde problemen ontstaan. Drie belangrijke vormen zijn acute mountain sickness (AMS), high altitude cerebral edema (HACE) en high altitude pulmonary edema (HAPE). AMS wordt vastgesteld aan de hand van de Lake Louise AMS-score en bestaat uit vijf symptomen die worden gescoord op aanwezigheid en ernst. Een score van ≥ 3 staat voor AMS. HACE is een encefalopathie met hersenoedeem en een toename van de intracraniële druk. HAPE is een vorm van hydrostatisch longoedeem op basis van een overdreven toename van de pulmonale vaatweerstand in bepaalde gebieden (hypoxische pulmonale vasoconstrictie) en daarmee de pulmonale bloeddruk. HACE en HAPE zijn ernstige ziektebeelden waaraan mensen kunnen overlijden.
