Hoofdstuk 8 Effecten van cardiale revalidatie op inspanningscapaciteit: invloeden van trainingsmodaliteiten

Samenvatting

Cardiale revalidatie leidt tot een belangrijke toename van de belastbaarheid. Hierbij dient men onderscheid te maken tussen de maximale en de aërobe belastbaarheid. Dit is belangrijk omdat deze twee vormen van belastbaarheid allebei een toename in capaciteit kunnen geven. De toename van de belastbaarheid is vooral het gevolg van perifere musculaire adaptatie en in mindere mate van centrale cardiale adaptatie. Voorspellers voor een groot trainingseffect van cardiale revalidatie zijn een lage belastbaarheid, uitgesproken perifere musculaire deconditionering en een grote capaciteit tot herstel van de myocardiale doorbloeding. Het is nog niet duidelijk of de linker ventriculaire ejectiefractie, de cardiale output en de mate van myocardiale ischemie bij de start van de revalidatie invloed hebben op de trainingseffecten. Factoren zoals ras, leeftijd, geslacht en het nemen van bètablokkers hebben geen invloed op de trainingseffecten. Een hogere trainingsintensiteit heeft geen invloed op de maximale belastbaarheid, wel op de aërobe belastbaarheid. Over de invloeden van de duur van de trainingssessies is niets bekend. Een langer programmaduur geeft een sterkere toename van de belastbaarheid, evenals een hogere trainingsfrequentie. Of extra krachttraining positieve effecten heeft op de maximale belastbaarheid, moet nog aangetoond worden. Er zijn echter wel positieve effecten van krachttraining op de aërobe belastbaarheid.
