9. Het primaire syndroom van Sjögren: huidige stand van zaken en ontwikkelingen

Abstract

Het syndroom van Sjögren (SS) is een auto-immuunziekte die aangrijpt op de exocriene klieren, in het bijzonder op de speeksel- en traanklieren. Het syndroom wordt onder meer gekenmerkt door een droge mond en droge ogen, die het directe gevolg zijn van een progressief verlies aan secretoir actief weefsel in de verschillende exocriene klieren (Kalk et al., 2002a; Fox, 2005). Als gevolg hiervan kunnen wisselende zwellingen van met name de glandulae parotideae optreden (in geval van een persisterende zwelling moet men bedacht zijn op een zogeheten mucosa associated lymfoid tissue (MALT-) of non-hodgkinlymfoom). Daarnaast klagen patiënten met het syndroom van Sjögren vaak over algehele malaise en chronische vermoeidheid (Meijer et al., 2009). Ook komen regelmatig extraglandulaire verschijnselen voor, zoals purpura, polyneuropathie en artritis (tabel 9.1) (Fox, 2005). Het syndroom van Sjögren wordt vooral bij vrouwen gezien, de vrouw-man ratio is 9 : 1. Sjögren kan zich op elke leeftijd openbaren, de mediane leeftijd ligt rond het vijftigste levensjaar. Hoewel Sjögren op elke leeftijd kan ontstaan, bestaat de indruk dat het syndroom tegenwoordig op een jongere leeftijd wordt gediagnosticeerd dan voorheen.
