Het nemen van medische beslissingen door patiënten met de ziekte van Parkinson

Samenvatting

Binnen de gezondheidzorg stelt het ethische principe ‘autonomie’ dat patiënten het recht hebben om zelf te beslissen over de zorg die zij ontvangen. Dit principe gaat er vanuit dat patiënten over de capaciteit beschikken om deze beslissingen te nemen. De ziekte van Parkinson is een progressieve, neurodegeneratieve aandoening die wordt gekenmerkt door motorische symptomen, maar ook door niet-motorische symptomen, zoals cognitieve en emotionele stoornissen. Als de ziekte voortschrijdt en de complexiteit van de behandeling toeneemt, zou de situatie kunnen ontstaan dat een patiënt met de ziekte van Parkinson verminderd in staat is om medische beslissingen te nemen. In zo’n geval zou het ethische principe ‘weldoen’ (d.w.z. het recht om behandeling en zorg te ontvangen die het beste is voor de patiënt) wellicht zwaarder moeten wegen dan het principe ‘autonomie’. De vraag is daarom of patiënten met de ziekte van Parkinson in staat zijn om medische beslissingen te nemen en hoe dit kan worden vastgesteld.
