Samenvatting

In dit hoofdstuk wordt het lichamelijk onderzoek van het hart beschreven. Om de betekenis van het onderzoek te begrijpen is kennis van de anatomie en fysiologie van het hart noodzakelijk. Het onderzoek is onderverdeeld in het standaardonderzoek en onderzoek op indicatie. Het standaardonderzoek omvat de inspectie, palpatie en auscultatie van het hart. Onderzoek op indicatie omvat aanvullende palpatie (kwaliteit ictus en thrills), percussie, aanvullende auscultatie (met membraan/kelk in verschillende houdingen) en beoordeling van de centraal veneuze druk. Bij de klinische betekenis van het hartonderzoek staat ook informatie over cardiale ziektebeelden met afwijkende bevindingen bij lichamelijk onderzoek. De volgende cardiale aandoeningen worden besproken: hartfalen, myocardinfarct en ischemie, hartklepafwijkingen, aangeboren hartafwijkingen en pulmonale hypertensie, pericardziekten, ritmestoornissen (extrasystolen, atriumfibrilleren, atriumflutter, AV-nodale re-entry tachycardie ofwel AVNRT, atriale tachycardie) en geleidingsstoornissen (AV-blok en rechter- en linkerbundeltakblok).
