Het effect van partnerrelaties op het geestelijk welbevinden van mensen met psychose: een systematische review

Samenvatting

Doel. Dat ondersteunende relaties een positieve invloed hebben op herstel van een psychose is inmiddels algemeen geaccepteerd, maar over hoe partnerrelaties hierin een rol spelen, is veel minder bekend. Deze review geeft een overzicht van kwantitatief onderzoek naar de invloed van partnerrelaties op de mentale gezondheid en het algemeen welbevinden van mensen met een psychotische stoornis.

Methode. Databases als PsychINFO, PubMed en Web of Science zijn systematisch doorzocht met zoektermen die verband houden met psychose en partnerrelaties. De selectie van studies is uitgevoerd door onafhankelijke beoordelaars. Verder is uitgebreid getoetst op kwaliteit en is een overzicht samengesteld in de vorm van een narratieve synthese.

Resultaten. Er zijn achtenvijftig studies geïncludeerd die rapporteren over het verband tussen partnerrelaties en respectievelijk psychotische symptomen, depressie, posttraumatische stressstoornis, suïcidaliteit, kwaliteit van leven, zelfvertrouwen en ‘satisfaction with life’. De resultaten leveren een gemengd beeld op, maar suggereren dat betrokkenheid in een partnerrelatie samenhangt met een vermindering van positieve en negatieve symptomen van psychose, maar ook met een toename van depressiviteit. In deze review worden mogelijke verklaringen voor die uitkomsten besproken, zoals het niveau van functioneren vóór de eerste psychose, de mate van sociale ondersteuning en de kwaliteit van de relatie.

Conclusie. De bevindingen wijzen op het belang van dit thema, een onderwerp dat in de klinische praktijk nog vaak over het hoofd wordt gezien. Er is dan ook behoefte aan wetenschappelijke studies die gericht zijn op het verkrijgen van meer inzicht in het verband tussen partnerrelaties en het welbevinden van mensen met psychose.
