Hardlopen is een populaire sport. In Nederland lopen meer dan 2,5 miljoen mensen regelmatig hard. Hardlopen is gezond en past goed in een verantwoorde leefstijl.
Met de toename van het aantal hardlopers stijgt echter ook het aantal hardloopblessures. Elk jaar raakt zo’n 20 tot 75% van de hardlopers geblesseerd. De grote spreiding heeft een aantal oorzaken: verschillen tussen de onderzoekspopulaties, de manier waarop blessures worden vastgesteld, de duur van onderzoeken en de follow-up.
De meeste hardloopblessures zijn overbelastingsblessures, zoals scheenbeenpijn, patellofemorale pijnsyndroom, patellapees tendinopathie, tractus iliotibialis-frictiesyndroom en fasciitis plantaris.
In de literatuur is geen consensus over de etiologie van hardloopblessures. Een overbelastingsblessure kan ontstaan als de cumulatieve belasting voor het houdings- en bewegingsapparaat groter is dan de belastbaarheid van een specifieke structuur. Zo bezien ontstaat elke hardloopblessure door trainingsfouten (too much, too soon).
Risicofactoren voor hardloopblessures zijn het trainingsvolume, eerdere blessures en competitief hardlopen. Er is geen relatie aangetoond met factoren als leeftijd, geslacht, body mass index (BMI), ondergrond, schoeisel en voettype.
Om (beginnende) hardlopers actief te houden zonder blessures, is goed gefundeerde informatie en advies nodig. Deze informatie is vaak moeilijk te vinden en wetenschappelijke onderbouwing ontbreekt veelal. Dit artikel overbrugt deze lacune met informatie over de huidige wetenschappelijke kennis rond de preventie van hardloopblessures.
