34. Genitale traumata

Samenvatting

In dit hoofdstuk worden verschillende oorzaken van genitale traumata besproken. In het eerste deel gaan we in op de penisfractuur, al dan niet met schade aan de omliggende weefsels. Dit letsel wordt meestal veroorzaakt door seksuele activiteit. De behandeling is operatief, behalve als het corpus cavernosum niet betrokken is (‘valse’ penisfractuur); in dat geval kan men conservatief blijven. Als tweede wordt ingegaan op paalletsels bij meisjes en jongens. Milde letsels kunnen door de huisarts behandeld worden. Ernstiger vormen moeten in het ziekenhuis beoordeeld worden door uroloog, gynaecoloog of chirurg. Het derde deel gaat over priapisme; een langdurige, ongewenste en pijnlijke erectie. Het priapisme is ischemisch indien vaso-occlusie de oorzaak is (low-flow priapisme) en non-ischemisch als er een continue bloeddoorstroming blijft bestaan (high-flow priapisme). Ischemisch priapisme dient met spoed naar de uroloog verwezen te worden. Als laatste wordt een corpus alienum in het genitaal beschreven.
