5. Genitale pijnklachten bij vrouwen

Samenvatting

Genitale pijnklachten bij vrouwen kunnen gelokaliseerd zijn in de vulva, vagina, baarmoeder, eierstokken en/of eileiders, en kunnen door een somatische aandoening veroorzaakt worden. Vulvodynie is een veelvoorkomende vorm van vulvaire pijn, al dan niet tijdens seksuele activiteit, waarbij een lichamelijke oorzaak is uitgesloten, dan wel de somatische aandoening niet de enige oorzaak is van de persisterende klachten. Bij vulvodynie ontstaat er een vicieuze cirkel waarin centrale sensitisatie (neuromodulatie) en perifere sensitisatie (neuroproliferatie) de pijnklachten onderhouden, naast afnemende zin en opwinding en toegenomen bekkenbodemspierspanning. In diagnostiek en behandeling is er aandacht voor de somatische, psychologische en relationele aspecten. Lichamelijke oorzaken worden uitgesloten, dan wel bevestigd. De vrouw (en partner) krijgt uitleg over de vicieuze cirkel en specifieke adviezen om deze te doorbreken: een (tijdelijk) pijn- en penetratieverbod, desensitisatie met vette zalf, gerichte exposure en relaxatie van de bekkenbodemspieren, en gesprekken en oefeningen gericht op het optimaliseren van de seksuele gezondheid.
