10. Genitale pijn bij mannen

Samenvatting

Genitale pijn en chronische bekkenpijn bij mannen omvatten een scala aan klinische uitingsvormen met een diversiteit en complexiteit aan etiologie. Dergelijke pijnklachten vragen om een zorgvuldige anamnese en diagnostiek vanuit het biopsychosociale perspectief, waarbij het noodzakelijk is een aanwezige partner actief te betrekken. Het is belangrijk chronische klachten van acute somatische klachten te onderscheiden. Daarnaast is het belangrijk seksualiteit expliciet te bespreken. Met name zullen hulpverleners sensitief moeten zijn voor eventueel bestaande seksuele trauma’s van jongens en mannen. Anodyspareunie, pijn bij receptieve anale seks, verdient meer aandacht als mogelijke oorzaak van chronische bekkenpijn. Somatische aandoeningen zijn vaak goed behandelbaar, maar het verdwijnen van de pijn geeft geen garantie voor herstel van de seksualiteit. Cognitieve therapie is nodig bij angst en vermijding, en traumabehandeling bij het verwerken van negatieve seksuele of soms medische ervaringen. Bekkenfysiotherapie kan effectief zijn bij een te gespannen bekkenbodem in aanwezigheid van genitale pijn en chronische bekkenpijn.
