13. Gemetastaseerde maligniteit bij onbekende primaire tumor

Samenvatting

Gemetastaseerde maligniteiten van onbekende origine vormen een uitdagend probleem in de kliniek. Bij 3–5 % van de patiënten met kanker worden metastasen vastgesteld zonder dat de primaire tumor gevonden wordt. Deze heterogene groep die voor het merendeel bestaat uit carcinomen heeft veelal een slechte prognose. Een klein deel van deze tumoren kan echter op basis van klinische criteria ingedeeld worden in een behandelbare subgroep, waarbij behandeling als zodanig resulteert in een significant betere prognose. Het diagnostische proces is dan ook gericht op het identificeren van deze subgroep. Essentiële onderdelen in de diagnostiek zijn anamnese en lichamelijk onderzoek, pathologisch onderzoek (immunohistochemisch en moleculair), serumonderzoek en radiologisch onderzoek. RNA expressie profielen en targeted therapy zullen mogelijk in de nabije toekomst bijdragen aan verbetering van de prognose van deze groep patiënten.
