Geïntegreerde psychologische en multidisciplinaire zorg

Samenvatting

Geleidelijk aan raakt men zich er meer van bewust dat het ontwikkelen van geïntegreerde psychologische en multidisciplinaire zorg een hoge prioriteit in de huidige gezondheidszorg behoort te hebben. Dat blijkt bijvoorbeeld uit dit themanummer, de publicatie in 2010 van het Handboek voor Multidisciplinaire zorg en de KNMG-handreiking ‘Verantwoordelijkheidsverdeling’.

Dit hoofdstuk is gebaseerd op twee hoofdstukken uit het boek Leentjens AFG, Gans ROB, Schols J, Weel C van (red.). Handboek voor multidisciplinaire zorg. De Tijdstroom, 2010. Namelijk: Huyse FJ, Weel C van, Latour CHM. De noodzaak van geïntegreerde zorg bij complexe patiënten; Huyse FJ, Latour CHM, Weel C van, Leentjens AFG. Complexiteit: screening en analyse.

In dit hoofdstuk komen aan de orde: a. wie vooral geïntegreerde psychologische en multidisciplinaire zorg nodig hebben: de complexe patiënten; b. uit welke elementen geïntegreerde zorg bestaat, welke elementen effectief zijn en hoe men met de INTERMED-methode de zorg voor complexe patiënten methodisch kan opzetten; en ten slotte c. met welke specifieke aspecten men bij de samenwerking rond complexe patiënten rekening moet houden.

Op dit moment is een pediatrische INTERMED in ontwikkeling.

Hier worden zowel klinisch geriaters, sociale geriaters als internisten met een aantekening geriatrie bedoeld.
