Geforceerd gebruik van de hemiplegische arm bij chronische cva-patiënten

Abstract

Bij meer dan de helft van de cva-patiënten die aanvankelijk een complete paralyse van de arm hadden en de acute fase overleven, blijft de aangedane arm a-functioneel, ondanks de gebruikelijke revalidatiemethoden (Nakayama et al. 1994). In een recent systematisch literatuuronderzoek werden dertien gerandomiseerde studies (randomized clinical trials, rct's) gevonden over het effect van oefentherapie voor de aangedane arm bij cva-patiënten (Van der Lee et al. 2001).

Line-bisection- test: patiënt krijgt een vel papier (formaat A4) recht voor zich waarop tien horizontale lijnen staan die niet gecentreerd zijn. Patiënt moet het middelpunt van de lijnen aanwijzen. Aan de hand daarvan kan gekeken worden of er een afwijking naar rechts dan wel naar links is.

Lettercancellation-test: patiënt krijgt vel papier (formaat A4) voor zich waarop allerlei letters staan. Het aantal o's aan de rechterkant is gelijk aan het aantal o's aan de linkerkant. Bedoeling is na te gaan of de patiënt dat ziet.
