Geestdrift voor het brein als belichaming van een foute verdeeldheid

Samenvatting

Neurobiologisering van de klinische psychologie en de psychotherapie is een hype, want niet gerelateerd aan reële wetenschappelijke bevindingen. Bovendien steunt deze hype op een achterhaald dualisme lichaam–geest, met als recentste vormgeving cognitie–affect. Echte vooruitgang moet gegrond worden op een overstijging van dit dualisme, maar die is nog niet in zicht.

Dit onderscheid is van belang, omdat het betekent dat volgens Freud de herinnering aan een trauma niet eerst bewust was en vervolgens weggeduwd (‘verdrongen’) naar het onbewuste. Integendeel, volgens Freud wordt het trauma onmiddellijk gefixeerd in ‘het systeem Onbewuste’ en bewerkt het van daaruit pathogene effecten. De verwarring met de eigenlijke of naverdringing (die bijvoorbeeld ook door LeDoux gemaakt wordt) is een latere bron van oneigenlijke kritiek.

Leuke woordspeling van Stephen Jay Gould (1981) op penis envy, waarmee hij de op minderwaardigheidsgevoelens gebaseerde jaloersheid bedoelt van de menswetenschapper op de fysicus, casu quo de harde wetenschappen. De woordspeling wordt nog interessanter met de stelling dat dergelijke binaire opposities inderdaad teruggaan op de oervorm man–vrouw (zie mijn laatste punt).

De meeste placebo’s zijn volledig inactieve stoffen, die de patiënt letterlijk niets doen voelen. Wanneer men een placebo gebruikt die een aantal onschuldige bijwerkingen veroorzaakt, dan voelt de patiënt inderdaad iets, is hij/zij ervan overtuigd dat ‘het’ werkt en blijkt het suggestieve placebo-effect exponentieel toe te nemen.

Als men de echt intrinsieke werking van een antidepressivum wetenschappelijk wil meten, dan kan daarvoor een eenvoudige methodologie toegepast worden: geef depressieve patiënten een antidepressivum zonder dat zij dit weten, en meet dan de eventuele verbetering. Dat dit ethisch gezien niet kosjer is, mag dan wel waar zijn, maar als dit argument van de farmaceutische wereld komt, klinkt het weinig geloofwaardig. Het is in ieder geval ethischer dan patiënten op een wachtlijst te plaatsen en hun behandeling gedurende x aantal maanden uit te stellen – en dat is vaak de praktijk.

Een aantal van die tijdschriften is inderdaad toonaangevend, in vakjargon: A1-tijdschriften. Bij zulke tijdschriften geldt een strenge blind peer review. Ondertussen heeft Big Pharma daar een handige oplossing voor gevonden: gesponsord onderzoek dat gepresenteerd wordt op een eveneens gesponsord congres, wordt als supplement (de proceedings) bij het tijdschrift geproduceerd (kosten daarvan worden andermaal betaald door Big Pharma). Op die manier wordt de blind peer review omzeild en krijgt de publicatie toch een wetenschappelijk aureool.

Dat betreft zowel het opzetten van het onderzoek zelf als het uiteindelijk uitschrijven van de resultaten. Dit wordt niet eens verborgen gehouden. Ter illustratie een advertentie uit de Vlaamse krant De Standaard van 30 november 2007 voor een ‘Senior Clinical Pharmacology Associate’ bij een van de grote Pharma-bedrijven. De vereiste essential functions zijn de volgende:
  • Initiation, monitoring and continuous reporting on assigned clinical trials in accordance with current GCP, procedural and local regulatory and ethical requirements.
  • Visits the study site frequently and collaborates closely with study staff to ensure optimal study conduct and to ensure follow up and communication of the critical safety and efficacy findings.
  • Assists in the writing/review of protocols and Clinical Study Reports where appropriate.
  • Develops and maintains links with investigators and research units in the therapeutic areas of importance for the company’s research.

In ons taalgebied is er het onthullende boek van Vandereycken en Van Deth (2006). In de New York Review of Books (8 juni 2006) vermeldt Marcia Angell onder andere de volgende boeken: Moynihan en Cassels, ‘Selling sickness’ (over hoe normale toestanden herlabeld worden als ziek, te behandelen met medicatie); Olfman, ‘No child left different’ (over de overpromotie en consumptie van psychoactieve farmaca bij kinderen); Kassirer, ‘On the take’ (over hoe artsen omgekocht worden); Abramson, ‘Overdosed America’ (over de overconsumptie van medicatie op basis van bewust gegeven verkeerde informatie betreffende de werkzaamheid ervan).
Ook politiek komt er beweging. Een Britse parlementaire onderzoekscommissie presenteerde hierover in 2005 een omvangrijk en zeer gedetailleerd rapport, met het volgende besluit: ‘De invloed van de farmaceutische industrie is dusdanig dat zij de klinische praktijk overheerst en dat er ernstige tekortkomingen zijn in de reguleringsmechanismen, met tot gevolg onveilig medicijnengebruik en toegenomen medicalisering van de maatschappij.’ (House of Commons, Health Committee, 2005)

Deze discipline omvat onder meer de studie van mechanismen die onder invloed van milieuomstandigheden een gen ‘aan’ of ‘uit’ kunnen zetten. In ruimere zin omvat zij onderzoek naar milieu-invloeden op de fenotypische uiting van ons erfelijk materiaal, waaruit blijkt dat tijdens het leven verworven kenmerken effecten kunnen hebben op de nakomelingen tot in de tweede generatie.
