Fixatie van het tertiusfragment in trimalleolaire fracturen

Samenvatting

Ondanks de relevantie van trimalleolaire fracturen is er nog veel discussie rond de beste behandeling ervan. Zowel biomechanische als klinische studies hebben geprobeerd helder te krijgen wat de beste behandeling is voor een fractuur van de posterieure malleolus (tertiusfragment) en daarmee het voorkomen van artrose op langere termijn. Biomechanische studies geven vooral aan dat schade ontstaat door verplaatsing van de contactdruk naar anterieur en mediaal. Klinische studies geven over het algemeen aan dat tertiusfragmenten groter dan 33% van het contactoppervlak gefixeerd dienen te worden. Met betrekking tot de behandeling en fixatie van kleinere fragmenten is er nog veel onduidelijkheid. Fixatie van deze fragmenten kan door schroeffixatie anterior-posterior of posterior-anterior (via een posterolaterale benadering). Verder onderzoek naar de langetermijneffecten van tertiusfragmenten en een RCT naar de behandeling van fragmenten kleiner dan 33% is nodig om meer licht te werpen op de juiste behandeling van trimalleolaire fracturen.
