Factoren die van invloed zijn op het verwijzen van patiënten van het verpleeghuis naar het ziekenhuis

Samenvatting

Introductie

Ziekenhuisopnames kunnen bij oudere patiënten negatieve gevolgen hebben. Het is daarom van belang dat het verwijzen van verpleeghuispatiënten voor een klinische opname weloverwogen plaatsvindt. Het doel van deze studie is te onderzoeken welke factoren hierbij een rol spelen.

Methode

Vragenlijstonderzoek onder specialisten ouderengeneeskunde en artsen in opleiding tot specialist ouderengeneeskunde.

Resultaten

Van de 1.540 verzonden enquêtes werden 200 enquêtes geretourneerd, waarmee een respons van 13 % werd bereikt. Ruim 60 % van de respondenten had in de afgelopen maand een patiënt van het verpleeghuis verwezen naar het ziekenhuis. Verblijf op een GRZ-afdeling, verdenking op een fractuur, een goede kwaliteit van leven, wens van patiënt of familie, een actief beleid en controleafspraken bij een specialist waren factoren waarbij verwijzing naar het ziekenhuis eerder werd overwogen. Intercollegiaal overleg en het aanwezig zijn van een klinisch geriater of internist ouderengeneeskunde worden bij insturen van belang geacht. Factoren die verwijzing van een patiënt minder waarschijnlijk maken zijn de diagnose dementie, een lage kwaliteit van leven en een symptomatisch beleid.

Conclusie

Zowel patiëntgebonden als niet-patiëntgebonden factoren spelen een rol bij verwijzingen van verpleeghuispatiënten. Verder onderzoek is gewenst om na te gaan of de factoren die meewegen bij het besluitvormingsproces ook leiden tot een juiste afweging.
