27. Extracranieel onderzoek: de carotiden

Inleiding

Het ultrageluidsonderzoek met kleurenecho-Doppler van de carotiden wordt voornamelijk gebruikt in de diagnostiek van cerebrovasculaire problematiek van allerlei aard. Het is een goedkope en patiëntvriendelijke methode om betrouwbaar een uitspraak te kunnen doen over het al dan niet aanwezig zijn van een hemodynamisch significante ofwel symptomatische stenose. Vaak – maar dit verschilt per kliniek – zal dit onderzoek gevolgd worden door conventioneel beeldvormend onderzoek om een precieze, driedimensionale indruk van de mate en de plaats van de stenose te krijgen. Tegenwoordig is de kwaliteit van het echo-B-beeld van dusdanig goede kwaliteit dat de morfologie van de vaatwand en eventuele plaque zeer nauwkeurig in beeld kan worden gebracht. De gevoeligheid van het ultrageluidsonderzoek in de diagnostiek naar significante vaatstenosen is dan ook vergelijkbaar met conventioneel beeldvormend onderzoek in de vorm van computed tomography angiography of magnetic resonance angiography (CTA/MRA). Ook is de driedimensionale echografie in opkomst, zodat wellicht in de nabije toekomst kan worden volstaan met het ultrageluidsonderzoek.
