Etnische verschillen in zwangerschap en verloskunde: uitkomsten en gedragsfactoren

Een overzicht van Nederlands onderzoek

Samenvatting

Als een mogelijke verklaring voor de ongunstige positie van Nederland inzake perinatale sterfte wordt gewezen op het grote aantal zwangere vrouwen met een niet westerse herkomst. In deze bijdrage wordt een overzicht gegeven van de voor Nederland beschikbare gegevens over etnische verschillen in perinatale en maternale sterfte, alsmede van etnische verschillen in voor zwangerschapsuitkomsten relevant gezondheidsgedrag en gebruik van verloskundige zorg. Het risico op zowel perinatale als maternale sterfte is vooral groot onder vrouwen met een sub-Sahara Afrikaanse herkomst (waaronder Creoolse Surinamers), gevolgd door Hindoestaanse vrouwen. Het verschil in perinatale sterfte tussen Marokkaanse en Turkse vrouwen enerzijds, autochtone vrouwen anderzijds is minder groot; wat betreft maternale sterfte zijn er geen verschillen. De perinatale en maternale sterfte onder vrouwen afkomstig uit andere westerse landen is juist lager dan onder de autochtone vrouwen. Sociaaleconomische positie lijkt deze etnische verschillen niet te verklaren. Onder Marokkaanse vrouwen is het aandeel dat (te) laat naar de verloskundige gaat het grootst; zij maken ook het minst gebruik van screeningen testmogelijkheden op het syndroom van Down. Tevens gebruiken zij het minst van al adequaat foliumzuur. Ook onder de Antilliaanse vrouwen komt een groot aantal (te) laat bij de verloskundige, rookt tijdens de zwangerschap en maakt relatief weinig gebruik van screeningmogelijkheden op het syndroom van Down. Turkse vrouwen vallen op door het hoge aantal dat rookt tijdens de zwangerschap. Gelet op de verschillen tussen allochtone groepen is het voor een Public Health beleid belangrijk om (deels) specifieke interventies te ontwikkelen. Voor onderzoekers is het – onder andere – belangrijk om ook aandacht te besteden aan nieuwe groepen immigranten, vooral deze uit sub-Sahara Afrika.
