14. Eosinofiele gastro-intestinale ziekten

Samenvatting

In de beginjaren negentig van de vorige eeuw werden de eerste gevallen van eosinofiele gastro-intestinale ziekten als aparte ziekte beschreven en ze worden de afgelopen jaren in toenemende frequentie herkend. Eosinofiele oesofagitis komt het meeste voor. Eosinofiele (gastro-)enteritis en colitis zijn zeldzaam, met uitzondering van door koemelk geïnduceerde allergische (procto)colitis bij jonge zuigelingen.

Eosinofiele infiltratie van de gastro-intestinale mucosa kan op de kinderleeftijd ontstaan bij ontsteking geassocieerd met hypersensitiviteit (IgE-gemedieerde allergie), gastro-oesofageale refluxziekte, infecties (parasitair), coeliakie, inflammatoire darmziekten (morbus Crohn, colitis ulcerosa), hypereosinofiel syndroom, als reactie op geneesmiddelen, graft-versus-hostziekte en auto-immuuncolitis. Indien voornoemde ziekten uitgesloten zijn en het ontstekingsinfiltraat in de lamina propria gedomineerd wordt door eosinofiele granulocyten, spreekt men van eosinofiele gastro-intestinale ziekten (EGIZ). In deze groep onderscheidt men eosinofiele oesofagitis (EoE), eosinofiele gastro-enteritis (EGE) en eosinofiele colitis (EC).
