30. Emboliedetectie met transcraniële Doppler

Inleiding

Herseninfarcten komen ten minste even vaak voor als hartinfarcten. Pathofysiologisch speelt embolisatie daarbij een voorname rol. De embolieën zijn afkomstig van het hart, de aorta of de grote, van de aortaboog afsplitsende vaten die de hersenen van bloed voorzien. De aanwezigheid van een bron van embolieën kan veelal worden aangetoond door detectie van micro-embolieën in de arteria cerebri media. Deze micro-embolieën zijn op zichzelf asymptomatisch maar geven wel aan dat er een emboligeen proces gaande is. Naast deze ‘vaste’ embolieën komen er ook gasvormige embolieën voor, bijvoorbeeld in samenhang met kunsthartkleppen of open hartoperaties. Al circa twintig jaar geleden beschreef Merrill Spencer voor het eerst dat het in een klinische context mogelijk was micro-embolieën in de a. cerebri media met transcraniële Doppler-sonografie (TCD) te detecteren. Dit hoofdstuk zal ten eerste de methode toelichten en ten tweede de toepassing van de emboliedetectie in de kliniek bespreken.
