32. Elleboogpijn (epicondylitis)

Samenvatting beleid

Bij elleboogpijn is meestal sprake van epicondylitis, met name epicondylitis lateralis. Het natuurlijk beloop ervan is gunstig, maar het kan wel lang duren (12–15 maanden). Bij elleboogpijn zijn er geen duidelijke alarmsignalen. De diagnostiek om een epicondylitis van een zenuwbeklemming te onderscheiden is lastig omdat de klachten en symptomen vergelijkbaar zijn. Bij het vermoeden van een fractuur, maligniteit of een andere organische oorzaak zou een röntgenfoto geadviseerd kunnen worden. Bij de diagnostiek van een epicondylitis, zenuwbeklemming of artrose is dit niet van meerwaarde. Geef uitgebreide uitleg: er is niets ernstigs aan de hand en het kan vanzelf herstellen. Voor een kortetermijnoplossing is het geven van een corticosteroïdinjectie een optie, alleen blijkt dit op de lange termijn mogelijk averechts te werken. De effectiefste conservatieve behandeling lijkt een multimodale behandeling te zijn, bestaande uit voorlichting, oefentherapie en eventueel manipulaties/mobilisaties. Oude behandelopties (van Cyriax) bestaande uit diepe dwarse fricties en een Mill’s manipulatie blijken geen aan te raden therapie; dit geldt ook voor behandelopties zoals ultrageluid, shock wave, spalken en tape. Het is zinvol om te verwijzen naar www.thuisarts.nl/tennisarm. Deel mee dat patiënten op internet allerlei therapieën tegenkomen. Indien deze niet zijn vermeld op bovenstaande website, zijn ze waarschijnlijk niet werkzaam.
